Sichrau Gwasanaethau Gofal Sylfaenol Meddygon Teulu Diogel a Chynaliadwy yng Ngogless Orllewin Powys: Mae’r ymgynghoriad yn mynd rhagddo rhwng 11 Chwefror 2019 a 28 Mawrth 2019.

Ensuring Safe and Sustainable GP Primary Care Services in North West Powys: Consultation is taking place from 11 February 2019 to 28 March 2019.

Mae Iechyd Bro Ddyfi, sy’n darparu Gwasanaethau Gofal Sylfaenol Meddygon Teulu i ryw 7,000 o bobl yng ngogledd orllewin Powys a’r cyffiniau, yn wynebu nifer o heriau wrth gynnal gwasanaethau lleol ar eu ffurf bresennol. Maen nhw wedi cyflwyno cais ffurfiol i Fwrdd Iechyd Addysgu Powys am gyfuno’u holl wasanaethau ar eu safle yn Forge Road, Machynlleth a chau eu safle yng Nglantwymyn erbyn mis Mehefin 2019.

Er mwyn ein helpu ni i ystyried y cais oddi wrth y practis a phenderfynu ar ddyfodol gwasanaethau lleol, rydyn ni’n ymgynghori â chleifion Iechyd Bro Ddyfi a’r cymunedau ehangach o amgylch Glantwymyn a Machynlleth.

Mae’r ymgynghoriad yn mynd rhagddo rhwng 11 Chwefror 2019 a 28 Mawrth 2019.

Rydyn ni’n gwybod bod cleifion a chymunedau lleol yn gwerthfawrogi eu gwasanaethau lleol. Felly, yr ydym wedi cyhoeddi dogfen ymgynghori sy’n manylu ar yr hanes, y sefyllfa sydd ohoni a’r opsiynau ar gyfer y dyfodol. Mae hyn yn cael ei anfon at gleifion sydd wedi'u cofrestru â Iechyd Bro Ddyfi i ofyn am eu barn, ac maent hefyd yn cael ei rannu â sefydliadau a rhanddeiliaid lleol.

Mae’ch barn chi’n hanfodol i’r broses hon.

Mae angen i ni ddeall sut y bydd unrhyw newid yn effeithio arnoch chi a’ch cymuned, a pha gamau y gallen ni eu cymryd i leihau effaith negyddol unrhyw newid. Rydyn ni’n eich annog i fynegi’ch barn trwy gwblhau a dychwelyd yr holiadur ymgynghoriad.

Nid oes angen ichi roi’ch enw, a bydd unrhyw adborth neu sylwadau yn gwbl gyfrinachol.

Bydd tîm ymgysylltu’r bwrdd iechyd yn dadansoddi’r holiadur. Mae’n bosibl y caiff ymatebion hefyd eu rhannu â Chyngor Iechyd Cymuned Powys, sef y corff annibynnol statudol sy’n cynrychioli buddion cleifion a’r cyhoedd yn y GIG.

Bydd eich barn yn cyfrannu at adroddiad a fydd yn hysbysu argymhelliad ar y ffordd ymlaen, a gaiff ei drafod mewn cyfarfod cyhoeddus o Fwrdd Iechyd Addysgu Powys yn ddiweddarach y gwanwyn hwn.

Nid yw’r Bwrdd Iechyd yn rhagfarnu’r canlyniad, ond os cymeradwyir y cais gan Iechyd Bro Ddyfi yna byddai eu safle yng Nglantwymyn yn cau erbyn Mehefin 2019.

Dyfi Valley Health, the provider of GP Primary Care Services for around 7,000 people in north west Powys and surrounding areas, is facing a number of challenges in maintaining local services in their current form. They have submitted a formal application to Powys Teaching Health Board to merge their services onto their Forge Road premises in Machynlleth and close their premises at Cemmaes Road by June 2019.

In order to help us consider the application from the practice and make decisions on the future of local services, we are consulting with patients of Dyfi Valley Health and the wider communities around Cemmaes Road and Machynlleth.

Consultation is taking place from 11 February to 28 March 2019.

We know that local patients and communities value their local services. So, we have published a consultation document which sets out the history, current situation and options for the future. This is being sent to patients registered with Dyfi Valley Health to seek their views, and is also being share with local organisations and stakeholders.

Your views are essential to this process.

We need to understand how any change would affect you and your community and what steps we could take to reduce any negative impact of any change. We encourage you to express your views by completing and returning the consultation questionnaire.

You don’t need to give your name and any feedback or comments will be entirely confidential.

We plan to hold a number of public meetings and drop in sessions where you can talk to the health board and other members of the community about this consultation. Find out more soon on our website atwww.powysthb.wales.nhs.uk/dyfi-valley-health

The questionnaire will be analysed by the health board’s engagement team. Responses may also be shared with the Powys Community Health Council who are the independent statutory body to represent the interests of patients and the public in the NHS.

Your views will contribute to a report which will inform a recommendation on the way forward, which will be debated in public at a meeting of the Powys Teaching Health Board later this spring.

The health board is not prejudging the outcome of this consultation, however in the event that the application from Dyfi Valley Health is accepted, then their premises at Cemmaes Road would close by June 2019.
