Gwobrau’r Gymraeg mewn Iechyd, Gwasanaethau
Cymdeithasol a Gofal Cymdeithasol 2015
Rydym yn chwilio unwaith eto am unigolion a grwpiau sy’n mynd yr ail filltir er mwyn sicrhau bod cleifion a defnyddwyr gwasanaeth yn derbyn gwasanaethau trwy
gyfrwng y Gymraeg fel rhan naturiol o’u gofal.
Mae’r Gwobrau’n agored i:
• Dimau, grwpiau a sefydliadau o fewn y GIG.
• Timau, grwpiau a sefydliadau o fewn Gwasanaethau Cymdeithasol.
• Sefydliadau gwirfoddol.
• Sefydliadau Addysg Uwch ac Addysg Bellach.
• Awdurdodau Lleol.
• Unigolion ar gyfer categori Dysgwr y Flwyddyn.
• Croesewir enwebiadau gan y cyhoedd ar gyfer pob categori.
Bydd y beirniaid yn chwilio am yr enghreifftiau gorau o ddarparu gwasanaethau dwyieithog gan ganolbwyntio ar effaith y fenter ar y defnyddiwr.
Byddant hefyd yn edrych am dystiolaeth o arweiniad cryf yn y maes hwn, sy’n mynd ymhellach na’r hyn a ddisgwylir yn y cynlluniau iaith ac enghreiffitiau o wireddu amcanion.
Y dyddiad cau ar gyfer derbyn enwebiadau yw’r 27 Mawrth 2015. NI DDERBYNNIR CEISIADAU HWYR. Bydd y Gwobrau’n cael eu cyflwyno yng Nghynhadledd y Gymraeg mewn Gofal Iechyd ar y 2 Gorffennaf 2015 ynVenue Cymru, Llandudno.

Words into Action
Welsh Language in Health,
Social Services and Social Care Awards 2015
The search is on again for groups or individuals who have gone the extra mile to ensure that patients and service users receive services through the medium of Welsh as a natural part of their care.
The Awards are open to:
• Teams, groups and organisations within the NHS.
• Teams, groups and organisations within Social Services and Social Care.
• Voluntary organisations.
• Higher Education and Further Education organisations.
• Local Authorities.
• Individuals for the Learner of the Year category.
• Nominations from the public are welcomed for all categories.
The judges will look for the best examples of providing bilingual service and will concentrate on the effect on the users’ experience.
They will also be looking for evidence of good leadership in this area of work, which goes further than what is expected under the Welsh Language Schemes and examples of putting the aims of into action.
The closing date for nominations is 27 March 2015. NO LATE ENTRIES WILL BE CONSIDERED. The Awards will be presented at the Words into Action Conference on 2 July 2015 at Venue Cymru, Llandudno.
