Meddygfa Trydydd Sector o wasanaethau a fydd wedi’u lleoli ger y feddygfa yn
ogystal รข rheoli prosiect " prescripsiwn cymdeithasol" gyda'r
ddwy feddygfa. Byddant yn cefnogi oedolion,
fel rhan o wasanaethau cymorth iechyd a llesiant integredig a leolir yn y
gymuned, i gael mynediad at wasanaethau a gweithgareddau yn y gymuned, a
gwasanaethau a ddarperir gan y sectorau cyhoeddus, preifat a’r trydydd sector,
trwy “frocera” mynediad at y gwasanaethau hyn a gweithio gydag unigolion i ddod
o hyd i atebion pwrpasol sy’n diwallu anghenion personol. Bydd y cydlynwyr yn cefnogi pobl i gael
mynediad at y cymorth cywir, y tro cyntaf, yn y ffordd gywir, a cheisio grymuso
unigolion i feithrin eu rhwydweithiau cymorth eu hunain yn eu cymunedau.

PAVO is seeking to recruit to the following posts:-
Two Community Health and Wellbeing Coordinators
Located in
Welshpool and Llanfyllin, working 35 hours per week.
Fluent oral and written Welsh is highly
desirable for these posts.
The post holders will coordinate a Third Sector Surgery of
services that will be based near the surgery location in addition project
managing “social prescribing” with both GP surgeries. They will support adults, as part of
integrated community based health and well-being support services, to access
local community services and activities provided by the public, private and
third sectors. Additionally, they will
broker their access to these services and work directly with the individual to
find bespoke solutions to meet their personal needs. They will support people to access the
right support, first time, in the right way and seek to empower them to build
their own networks of support within their community.
Salary:- £24,717
per annum funded until 31st
March 2018
