Parents/Carers Forum Survey




Please find attached a briefing note which explains how we are hoping to establish a Forum for Parents/Carers of children/young people with disabilities/complex health conditions.

 We are looking for organisations and parents/carers to register interest, state how much you would like to be involved and how best to communicate with you. 

 Please would you circulate this to all your networks giving as many parents/carers as possible to become involved.
