Children & Young People's Partnership Multi-Agency Meeting: TRANSITIONS





PAVO want to represent your views to help shape services.

 This meeting on Thursday 25th June focuses on TRANSITIONS.  

 What does it mean?  

 What are the key points of Transitions? 

 What are the top 3 Transitions Points that need further work?  
PLEASE let Yvonne at PAVO have any case studies, issues, stories, views that you have around the experiences of children & young people and their transitions to help with the discussion and help to shape future services.
