Expression of Interest
PAVO is
pleased to be involved in a new project aimed at supporting and developing
integrated care for older people. Welsh
Government has made one off funding available for 2014 – 2015 to take forward
integrated care for older people on a regional basis. This fund – The Intermediate Care Fund – is administered through the local
authorities with Powys being part of a consortium with Ceredigion, Carmarthen
and Pembrokeshire.
In Powys, the lead body for overseeing this, ICPOP
(Integrated Care Pathway for Older People), has agreed a portfolio of projects
to address different identified needs.
Project Title: ‘Services for Independent Living’
Overall
Aim: “To enable older people to sustain or regain independent
living through the provision of new or enhanced practical services delivered by
the third sector.”
